Work Environment\nThis position is primarily remote, allowing for a flexible work arrangement while maintaining access to our comprehensive outpatient office facility in Woodrow, Colorado. Although the majority of the work may be conducted from home, occasional in-office meetings and team collaboration sessions will be required to ensure the delivery of effective integrated care. Standard working hours are Monday through Friday, from 8:00 AM to 5:00 PM, with some evening shifts as needed to accommodate patient schedules.
Key Responsibilities\n- \n
- Conduct in-depth mental health assessments that consider the interplay between psychiatric and medical conditions, identifying the unique needs of each patient. \n
- Develop and implement comprehensive treatment plans that are both tailored and integrative, effectively addressing psychiatric issues as well as underlying medical conditions. \n
- Facilitate individual and group therapy sessions, employing evidence-based therapeutic techniques such as cognitive-behavioral therapy, mindfulness, and motivational interviewing. \n
- Engage in active collaboration with the interdisciplinary team, including physicians and nurses, to ensure alignment and cohesive treatment strategies for each patient. \n
- Provide timely crisis intervention, offering support during mental health emergencies, and maintaining the emotional safety of our clients. \n
- Educate patients and their families about the relationship between mental and physical health, equipping them with strategies to manage their conditions effectively. \n
- Ensure accurate and confidential patient documentation, adhering to all legal and ethical standards in mental health practices. \n
- Participate actively in team meetings and case conferences to refine treatment practices and share insights about patient care. \n
- Stay informed on the latest research developments and best practices in integrated mental health care and apply this knowledge to your practice. \n
This role will focus on serving a diverse patient population grappling with co-occurring mental health and chronic medical conditions, including chronic pain, autoimmune disorders, and substance use disorders. This unique blend of challenges requires a high level of skill, empathy, and collaboration to ensure optimal recovery and quality of life for our clients.
Qualifications\n- \n
- A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is required. \n
- Current licensure as an LCSW, LPC, or LMFT in the state of Colorado is mandatory. \n
- Proficient experience in addressing the mental health concerns of individuals with medical complexities. \n
- Previous experience in delivering integrated care and collaborating with medical professionals is essential. \n
- Strong interpersonal and communication skills to effectively interact with patients and colleagues. \n
- Commitment to maintaining professional boundaries and adhering to ethical guidelines, ensuring patient confidentiality at all times. \n
- Familiarity with electronic health record (EHR) systems and related healthcare applications is preferred. \n
- A collaborative mindset, demonstrating a readiness to be part of an interdisciplinary team dedicated to patient care. \n

The salary range for this position is competitive, offering between USD 75,000 and 104,000 annually, commensurate with experience and qualifications.
