Community Physical Therapy (CPT) is looking for a motivated Physical Therapist (PT) to join our Admit-to-Discharge team! If you enjoy having the freedom to manage your schedule while guiding patients from their initial evaluation through discharge, this could be the opportunity you?ve been looking for.

Monday?Friday scheduling between 7:00 AM and 6:00 PM
No weekend requirement
Variety in your day with outpatient and home care patients
Continuity of care ? follow your patients from evaluation through discharge

What We?re Looking For
Active Illinois Physical Therapist (PT) license
Outpatient or home care experience is a plus
Dependable, flexible, and comfortable working independently
Passion for patient-centered care and helping patients achieve their goals
