In this role, you will evaluate residents, develop individualized treatment plans, and provide therapy services that help residents achieve their highest level of independence and quality of life.Key ResponsibilitiesEvaluate residents and develop individualized treatment plans based on physician orders and clinical needsProvide skilled occupational therapy services in accordance with established goalsMonitor resident progress and modify treatment plans as appropriateCommunicate effectively with physicians, nursing staff, residents, families, and interdisciplinary team membersDocument evaluations, treatment sessions, progress notes, and discharge summaries accurately and timelyParticipate in care planning meetings, rehabilitation conferences, and discharge planningEducate residents, families, and caregivers on treatment goals and home programsConduct in-service training and educational programs for facility staffEnsure compliance with all state practice acts, regulatory requirements, and facility policiesPromote resident independence, safety, and overall well-beingQualificationsEducation & ExperienceBachelor's degree in Occupational Therapy requiredMaster's or Doctorate degree in Occupational Therapy preferredExperience in skilled nursing, rehabilitation, or long-term care preferredExperience with PointClickCare (PCC) and Casamba preferredStrong communication, documentation, and organizational skillsLicenses & CertificationsCurrent South Carolina Occupational Therapist license in good standingMust maintain all required continuing education requirementsMust remain in good standing with the State BoardApply TodayJoin a therapy team dedicated to helping residents regain function, improve independence, and enhance their quality of life.
