Key Responsibilities\n- \n
- Conduct thorough patient intakes focusing on diagnostic and functional evaluations, providing insightful recommendations to primary care providers regarding treatment goals. \n
- Deliver brief and targeted therapeutic interventions for patients seeking mental health services, helping them navigate through emotional challenges. \n
- Assess which patients can be effectively treated within the primary care setting and identify those requiring referrals to specialized mental health facilities. \n
- Engage in proactive follow-up communications with patients to track progress, adjusting treatment plans as necessary to improve outcomes. \n
- Develop and implement relapse prevention strategies to help patients maintain stable mental health and avoid setbacks. \n
- Monitor and coordinate behavioral health services, ensuring patients have access to additional resources as needed. \n
- Thoroughly document patient interactions, progress, and treatment plans in compliance with established guidelines and organizational policies. \n
- Work cooperatively with interdisciplinary teams to promote behavioral changes and healthier lifestyle choices among patients. \n
- Consult with clinic management and team members on behavioral health services to inform and enhance program evaluation and outcomes. \n
- Adhere to ethical standards and maintain confidentiality in all patient-related matters. \n
Our patients include individuals from diverse backgrounds and socioeconomic statuses within the San Tan Valley area. Many face mental health challenges stemming from various socio-environmental pressures. Qualifications\n- \n
- Master’s Degree in Clinical Counseling or a related field. \n
- Current LPC (Licensed Professional Counselor) certification in Arizona. \n
- In-depth knowledge of mental health assessment, treatment planning, developmental psychopathology, and psychotherapeutic techniques. \n
- Ability to evaluate the individual needs of patients and establish strong therapeutic rapport. \n
- Excellent communication skills, both verbal and written, to effectively collaborate with patients and healthcare professionals. \n
- Strong organizational skills and the ability to maintain accurate and up-to-date clinical records. \n

- Bilingual proficiency in English and Spanish is preferred, facilitating better communication with a wider patient demographic. \n
- Experience working in community health settings or Federally Qualified Health Centers (FQHC) is a plus. \n
- Familiarity with electronic health record systems to document patient care efficiently. \n
The yearly salary range for this position is between USD 87,000 and 105,000, commensurate with experience and qualifications.
