Physical Therapist

Job Summary The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. Duties & Responsibilities Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s). Provides patient, caregiver, and family counseling. Provides patient and caregiver education. Prepares clinical notes and progress summaries based on attainment of goals. Communicates with the physician responsible for the home health plan of care and other healthcare practitioners related to the current home health plan of care. Provides physical therapy services to patients according to a written physician’s plan of care, including: Assessment and evaluation of therapeutic/rehabilitative/functional status and participation in development of the total plan of care; Evaluation of home environment; Assessment of muscle strength, mobility, gait, ROM, and potential for rehabilitation; Directing physical therapy treatment; Instructing patients and families/caregivers in use and care of therapeutic appliances; Determining priority needs for physical therapy; Reporting to physician patient’s reaction to treatment or changes in condition; Initiating physical therapy program and instructing other personnel, family, or caregiver members in certain phases of therapy, including goals through case conferences; Training patients in use of prosthetic devices as needed; Identifying patient and family/caregiver needs for other home health services and referring as necessary; Participating in discharge planning for patients; Providing in‑service education programs for nursing organization personnel as needed; Participating in peer consultation processes; When therapy is the only skilled service, instructing, supervising, and evaluating home health aide care every two weeks; Supervising Physical Therapy Assistants according to organization policy and state regulations; Participating in the HHA’s quality assessment and performance improvement program and HHA‑sponsored in‑service training. Inspects and documents patient progress and reports to physician and care team. Participates in case conferences and interdisciplinary team meetings. Provides in‑service education to nursing organization personnel as needed. Engages in peer consultation and quality assessment activities. Performs supervisory responsibilities for PTAs and home health aides when therapy is sole skilled service. Job Requirements Licensed to practice as a physical therapist within the State, if applicable. Passed an examination for physical therapists approved by the state and possesses a degree from a baccalaureate or master’s program in physical therapy approved by CAPTE or successor organization. Two years of appropriate experience as a physical therapist. Community/home health experience is preferred. Demonstrates strong verbal and written communication and organization skills. Maintains current CPR certification. Licensed driver with an automobile that is insured and in good working order.
