Job Summary
We’re looking to add to ourextraordinary care team. Grounded by our belief that outstanding care is bestdelivered in a team-based environment, our Home Health Physical Therapistscollaborate to provide management and delivery of patient care plans.
In partnership with the RehabilitationHospital, our organization is an accredited recognized leader in home health.We deliver industry-leading therapy and nursing expertise and services thatexceed national benchmarks throughout the West Michigan service area.
This position supports patients in Paw Paw and surrounding areas and is eligible for a $10,000 first year successbonus to reward the accomplishment of key achievements during your first year!
Requirements
Bachelor’sdegree or Master’s degree in Physical Therapy from an accredited college/university
Mustbe a Licensed Physical Therapist in the state of Michigan
Minimumof one year healthcare experience
Currentdriver’s license and ability to spend ~20% of your day driving to/from patientlocations
Acommitment to consistently meet critical deadlines for charting
Theskills needed to self-manage your time and schedule
Demonstratedexperience with tablets, mobile phones and EMR software
