Job Details:

• Title: Physical Therapist (PT)
• Employment Type: Permanent / Full-Time
• Location: Oakland, CA 94611
• Schedule: 8:30 AM – 4:30 PM
• Compensation: $58–$62/hour (Based on experience) + Benefits

• Skilled Nursing Facility (SNF)
• Post-Acute Rehabilitation

-> Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled PT interventions to improve mobility, strength, balance, coordination, and functional independence.
• Treat patients with a variety of conditions including orthopedic, neurological, and medically complex diagnoses.
• Monitor patient progress and adjust treatment plans based on patient needs.
• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and interdisciplinary care teams.
• Complete accurate and timely clinical documentation.
• Educate patients and caregivers on therapy goals, mobility techniques, and home exercise programs.
• Participate in care planning and maintain compliance with Medicare, state regulations, and facility policies.

-> Qualifications

• Active California Physical Therapist (PT) license .
• Graduate of an accredited Physical Therapy program (DPT preferred).
• Experience in Skilled Nursing Facility (SNF), Long-Term Care (LTC), Rehab, or Post-Acute Care preferred.
• Strong clinical judgment, communication, and documentation skills.
• CPR/BLS certification preferred.
