Job Description At CareOne, our Social Workers play a vital role in supporting residents, families, and the interdisciplinary care team to ensure the highest level of emotional, psychosocial, and discharge planning support throughout the resident stay. As a Social Worker with CareOne, you'll work collaboratively with nursing, rehabilitation, admissions, and leadership teams to help residents navigate complex healthcare needs while maintaining dignity, independence, and quality of life. Salary Range: $55,000 - $70,000 (Full-Time) Practice Settings:
- Short-Term Rehabilitation

- Participates in an interdisciplinary team approach to support resident care, discharge planning, and overall quality outcomes.
- Provides therapeutic intervention and emotional support to residents and families coping with illness, disability, institutionalization, or life changes.
- Completes psychosocial assessments and maintains timely, accurate documentation in accordance with company policy and regulatory requirements.
- Develops and implements individualized social service care plans with measurable goals and interventions.
- Participates in interdisciplinary care plan meetings and ensures resident and family involvement in the care planning process.
- Acts as a liaison between residents, families, facility leadership, and outside agencies to support resident rights and advocacy.
- Conducts family meetings, support meetings, and other resident-centered communication initiatives as needed.
- Facilitates discharge planning and coordination of services to support safe and successful transitions of care.
- Assists with admissions processes including referral coordination, insurance verification, and facility tours when needed.
- Develops and maintains positive relationships with referral sources, healthcare partners, and community organizations.
- Supports a safe environment for residents, visitors, and staff while following all center policies and procedures.
Position Requirements
- Master's Degree in Social Work or related human services field.
- Current CSW or LSW licensure within the state of operations.
- Minimum of one year of Social Work experience preferred.
- Long-term care or skilled nursing experience preferred.
- Strong communication, organizational, and interpersonal skills.
- Ability to work collaboratively within an interdisciplinary healthcare environment.
