Social Work Care Manager - Palliative Care

Job Description /n Job Description: The Social Work Care Manager I utilizes clinical expertise to perform psychosocial assessments, develop and implement care plans in collaboration with the appropriate care team, and assess crisis situations to provide clinical counseling, diagnosis, brief therapeutic interventions, and necessary resources or referrals. This role also includes providing individual and family treatment as indicated. The position works collaboratively with patients, their support persons, healthcare providers, insurers, community resources, and all other involved parties./n Palliative Care - LCSW Required /n M-F 8a-4:30p /n /n This is a unique position wherein the Palliative Care Social Work Care Manager will partner closely with the Palliative Care Team at St. Vincent Hospital to provide high-quality, patient-centered care in the acute hospital setting. /n /n Essential Functions /n /n - Assessment & Screening: Evaluates patients for transition planning, mental health, substance use, and goals of care. /n - Care Coordination: Develops and monitors care plans, addressing social determinants of health and community resources. /n - Behavioral Health Support: Uses motivational interviewing and therapeutic techniques to promote mental health care including women’s services, behavioral change, trauma informed care, and substance use disorders. /n - Therapeutic Intervention: Provides brief individual, group, and family therapy, plus psychosocial assessments. /n - Diagnosis & Referrals: Identifies mental, emotional, and behavioral disorders and connect patients to services. /n - Education & Advocacy: Trains staff, educates patients, and advocates for rights and care access through facilitating safe transitions of care to the community. /n - Team Collaboration: Works with healthcare teams, insurers, and community providers for quality care. /n - Quality & Compliance: Leads improvement initiatives, tracks key metrics, and ensures policy adherence. /n - Advanced Care Planning: Facilitates clinical goals of care discussions with patients, families, and teams. /n /n /n Skills /n /n - Clinical Therapy Techniques /n - Care Planning /n - Motivational Interviewing /n - De-escalation & Problem Solving /n - Trauma-Informed Care /n - Critical Thinking /n - Time Management /n - Patient Education /n - Communication /n - Prioritization /n /n Minimum Qualifications /n /n - Master of Social Work (MSW) from an accredited institution (degree verification required). /n - Current state licensure, as applicable, is obtained prior to or upon completion of required supervision hours. /n - Basic computer proficiency, including familiarity with word processing and spreadsheet software. /n - Strong written and verbal communication skills. /n - Demonstrated ability to apply critical thinking skills. /n - /n Caregivers whose duties require them to conduct home or community visits must maintain current BLS certification, have a current driver’s license, current auto insurance, an acceptable driving record and reliable transportation. /n /n /n Preferred Qualifications /n /n - Case Management Certification. /n - Experience in clinical care management, social work, or working with third-party payers. /n - Demonstrated understanding of care management principles and practices. /n - Demonstrated understanding of health insurance products and related processes. /n - Demonstrated understanding of coding, episode of care, and length of stay guidelines. /n - Ability to work independently, demonstrate self-motivation, maintain a positive attitude, and adapt to a rapidly changing environment. /n /n /n Physical Requirements /n /n - Ongoing need for employee to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs. /n - Frequent interactions with providers, colleagues, customers, patients/clients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately. /n - Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc. /n - May have the same physical requirements as those of clinical or patient care jobs, when the leader takes clinical shifts. /n - For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles. /n /n /n Location: Intermountain Health St Vincent Regional Hospital/n /n Work City: Billings/n /n Work State: Montana/n /n Scheduled Weekly Hours: 40/n /n The hourly range for this position is listed below.
