Job Title: Unit Manager - LPN (Effective Date: 01/01/2017) Department: Nursing Reports to: Director of Nursing Services Supervises: All Nursing Staff General Description Under the supervision of the Director of Nursing Services, the Unit Manager is responsible for the overall management of residents on a designated unit. The role directs, coordinates, and supervises nursing staff activities and transmits each unit’s staff concerns to the appropriate department supervisor. The manager must be able to perform all physical and mental job functions of a Certified Nursing Assistant and Licensed Practical Nurse. Essential Duties Perform various duties to provide quality nursing care to residents, including: Plan effective administration of nursing care in cooperation with other health‑related disciplines. Observe and report resident needs, develop and implement comprehensive care plans, consult with physicians, and periodically review and update care plans. Evaluate incidents and determine emergency measures, monitor and direct emergency treatment, notify physicians as needed, complete incident reports, and update care plans with new interventions. Supervise planning of daily assignments for personnel on the unit. Ensure adherence to established policies and procedures and maintain staff awareness. Schedule and lead staff conferences to promote initiative and resolve resident‑care problems. Collaborate with interdisciplinary team members to develop discharge plans for residents. Assist in orienting and directing new staff. Maintain accurate records of resident care and related statistics. Review performance against accepted standards, conduct performance evaluations, and complete proficiency audits for nurses and CNAs. Direct requisitions of equipment and supplies, encouraging proper utilization. Provide input to the In‑service Coordinator for nursing in‑service programs and unit conferences. Handle resident or staff grievances or refer them to the appropriate supervisor. Maintain positive staff relations and public image. Be available for emergencies and assist nursing staff as required. Serve as liaison among residents, families, physicians, and staff. Accompany physicians on rounds and treatments as needed. Promote and maintain safe work performance by staff. Communicate pertinent information concerning resident status, staff actions, and suggestions to the Nursing Supervisor and Director of Nursing Services. Conduct resident rounds to monitor nursing care and promote a safe, clean, comfortable environment. Receive shift reports and verify continuity of care. Participate in Nursing Management Meetings, HART Team Meetings, and Comprehensive Care Plan Conferences. Provide input in formulating nursing policies and procedures. Notify residents’ physicians, families, or health‑care agents of any change in resident condition. Establish and maintain resident care systems per standardized procedures, including: Unit Management System for group assignments and nursing assistant responsibilities. Restraint Reduction System to verify minimal restraints, with releases every two hours or per care plan. Turning and Positioning System to verify schedules are followed every two hours or per care plan. Resident Ambulation System to verify residents adhere to their ambulation program. Scheduling system to verify fair and consistent scheduling for nurses and CNAs. Nourishment/Supplements System to verify timely passage of nourishments and supplements. Cleanliness System to verify a clean, orderly environment and daily cleaning schedules. Infection Control System to ensure proper infection control protocols. Pharmacy System to verify timely receipt and proper disposal of medications, monitor medication rooms and carts. Laboratory System to verify timely draw and ordering of labs. Review Change of Condition/24‑Hour Report daily and adjust resident status as needed. Standard Requirements Support the Mission, Values, and Vision of the Facility. Protect resident rights and uphold an environment of privacy, dignity, and well‑being. Implement procedures and programs for OSHA and safety, fire/safety/disaster plans, risk management, and security; report and correct unsafe conditions. Adhere to confidentiality, HIPAA, quality assurance, regulatory compliance, workplace anti‑harassment, and the Code of Conduct. Participate in teamwork by cooperating with co‑workers, completing duties with minimal supervision, using appropriate judgment, and communicating tactfully and follow‑up on concerns. Immediately report incidents of alleged resident abuse, neglect, or rights violations to the Director of Nursing or Executive Director. Be knowledgeable of developmental factors for adult and geriatric residents in care planning. Maintain current practice standards through participation in educational opportunities on and off site. Other Duties Provide hands‑on nursing care to residents as needed, consistent with the state’s Nurse Practice Act. Perform supportive tasks that may be adapted to circumstances.
