Responsibilities
Assess and treat patients with PT plans based on individual goals.
Develop and implement evidence-based treatment plans with measurable milestones.
Monitor patient progress and adjust therapies to optimize outcomes.
Maintain licensure, adhere to professional standards, and ensure patient safety.
Document patient encounters accurately in electronic health records.
Collaborate with a multidisciplinary team to align on care plans.
Contribute to monthly targets and pipelines by documenting potential patient referrals.
Educate patients and caregivers on exercises and home programs.
Identify and communicate barriers to progress and adjust plans accordingly.
Participate in continuous learning and professional development activities.
Support on-site operations, ensuring efficient patient flow and wait times.
Experience/Qualifications
Licensed Physical Therapist with active state license and clean practice record.
Develop and implement evidence-based PT plans with measurable milestones.
Regular progress monitoring and therapy adjustments to optimize outcomes.
Meet monthly targets and contribute to pipeline by documenting potential referrals.
On-site patient flow optimization to minimize wait times and maximize throughput.
Accurate documentation of patient encounters in electronic health records (EHR).
Collaborate with multidisciplinary team to align on care plans and referrals.
Identify barriers to progress and adjust plans accordingly.
Educate patients and caregivers on exercises, home programs, and safety.
Commitment to ongoing professional development and participate in training.
