Job Summary

We are looking for individuals to join our client’s team as Case Worker. Under supervision, provides social services to clients/patients, such as; recipients of public assistance, food stamps and/or medical assistance, adults receiving or needing institutional care or protective services. Identifies, develops and implements social service plans for such families/individuals – determine the eligibility for these services and may perform counselling and investigatory activity related to the work. 

Job Description

• Send facility placement referrals
• Follow up with nursing home liaisons and interdisciplinary staff to secure appropriate placements
• Reply to requests for additional information
• Keep statistical and progress reports
• Attend ALOC meeting and provide updates
• Take steps to overcome barriers to placement
• Send out timely referrals, follow on requested information, and respond to social work staff regarding outcomes

Skills Required

• 3 years’ experience in social services, case management, or related field.
• Knowledge of social services, with excellent verbal and written communication skills and strong computer literacy.
• Preferred: Hands-on experience with EPIC and/or Careport.

Education/Training/Certifications

• Bachelor’s Degree required from an accredited college or university

Additional Requirements

• Shift Time: 9:00 AM-5:00 PM; Monday - Friday
• This is onsite role. Home visits are not needed.

  Pay Rate

• $37–$43 per hour, based on experience, skills, and market factors
