Description:

Dependable Nursing is seeking an experienced contract Physical Therapist to join our growing home health team serving South San Diego County including Chula Vista, Otay Mesa, National City, and surrounding communities.

Position Details

Contract per visit — flexible schedule
South San Diego County service area
Consistent case flow with opportunity to grow your caseload

Responsibilities

Conduct skilled physical therapy evaluations and treatment visits in the patient's home
Complete and submit OASIS assessments accurately and on time
Develop and implement individualized plan of care in coordination with the physician and care team
Provide gait training, strengthening, balance, transfer training, fall prevention, and ADL retraining
Educate patients and caregivers on home exercise programs and safe functional mobility
Maintain timely documentation in compliance with Medicare and agency standards
Communicate clinical updates and changes in patient condition to the clinical team

Requirements:

Active California Physical Therapy license in good standing
Minimum 1 year of experience
OASIS competency required — must be able to complete independently
CPR certification current
Reliable transportation and valid California driver's license
Strong documentation and communication skills
