The work is sometimes challenging and always rewarding by engaging with persons we serve and co-workers as we all learn, grow, and thrive.Position Overview:As a member of the multi-disciplinary Community Support Services team, provides resource development, skill development and direct support to assist people who have serious mental illness who are enrolled in CSS to fully integrate into the community of their choice.Essential Job Functions:Create and Maintain:A wellness and recovery-oriented environment by using person-first language, respectful documentation, person-centered planning, and evidence-based practicesA culture of compliance with internal and external policies, regulations, laws and high ethical and professional standards.Community Support Services Responsibilities:Contributes to the Comprehensive Rehabilitation Needs Assessment on an ongoing basis.Participates with the licensed staff in the development, implementation, and monitoring and updating of the individualized rehabilitation plan/Individual Recovery Plan (IRP).Perform therapeutic rehabilitative skill development as outlined in the CSS Regulations.Offer services appropriate to meet person's served needs as identified in the Comprehensive Rehabilitation Needs Assessment (CRNA) and included in each individuals IRP.Perform crisis intervention as indicatedCoordinate and manage services in conjunction with the team.Supportive counseling, medication education, mental health education and early crisis interventionEngages people receiving services in the treatment process.Utilizes Illness Management and Recovery, WRAP and other relevant health and wellness curriculum to ensure overall wellness is a key focus in the individual's recovery planPerform evidence-based practices that are appropriate for implementing community support services. This may include Motivational Interviewing, Cognitive Behavioral and Educational Strategies, Trauma Informed Care, Career Services, Permanent Supportive Housing, IMR and WRAP.Mental health education, community living skill training, and supportive counseling in the home and other natural environments to person served and significant others24-hour on-call coverage on a rotating scheduleTechnical assistance and education to members of the team in the area of recovery and wellness. Works with the team to develop materials and strategies for teaching wellness strategies including Wellness Recovery Action Plans, instilling hope, promoting choice and empowerment, and spirituality, in conjunction with the teamServes as a resource for all team members to ensure health literacy principles are translated into services and supports offered for all persons served, especially those who are at risk for metabolic syndromeConsults with other service providers and resource people regarding the assessment, treatment and rehabilitation needs of the people receiving servicesCompletes chart documentation for assigned individuals according to CSS regulationsRequirements:Education and Experience:Associate degree in one of the helping professions, such a social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice, from an accredited college or university and will have a minimum of two years of experience in a community-based behavioral healthcare setting.Certification as a psychiatric rehabilitation practitioner may be substituted for one year of experience.Staff employed under this subsection cannot supervise others and shall work under the supervision of a licensed physician, psychiatrist, psychologist, advanced practice nurse, registered nurse, or master's level community support staff.Bachelor's degree from an accredited college/university in one of the helping professions, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice.Master's degree from an accredited college/university in one of the helping professions, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice.Individuals with a Bachelor's degree only in a field other than a helping profession will have a minimum of two years of professional experience in a community behavioral health care setting.Certification as a Psychiatric Rehabilitation Practitioner may be substituted for one year of experience.Driver's License, Vehicle:Valid driver's license requiredNo more than one moving violation within the past 12 monthsVehicle requiredExcellent Benefits:Benefits: Medical, Dental, Vision, 403b, basic life and AD&D, flexible spending accounts, EAPEligible for medical benefits after 30 days of employmentFlexible work schedules, clinical training series, leadership development program10 paid holidays (an 11th after 2 years of employment), generous vacation and sick timeBridgeway Behavioral Health Services was founded on a strong desire to fight disparity and injustice alongside people living with behavioral health conditions.
