RN - Home Health

Registered NurseProvides coordinates and directs the provision of home nursing care according to physicians' orders. This is based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits are required. Periodic office meetings will be necessary. Must have reliable transportation and current driver's license. Agency-required liability insurance is needed. Average number of patients per day: 5-8. Points per week: 30 points per week are expected. Each type of visit and activity (documentation, travel, etc.) holds a different point value. Contractor will be assigned between 5-10 patients per day, which equates to roughly 6-8 points. If there is a situation where the contractor is unable to meet their financial needs due to low patient census, please reach out for assistance. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Traveler is compensated from patient home to patient home. The commute to and from the first and last patient are not billable. Local candidates are accepted at the same rate. Home Health experience is required. The candidate will be responsible for driving to patients' homes daily and to the clinic for weekly meetings. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation will impact billing and follow-up patient care. The clinician must hit weekly productivity points of 30, which equates to 40 hours per week. All overtime must be pre-approved. If patients are not available each week to support financial needs, there will be an opportunity to float to another clinic to pick up patients. All travelers must be proficient in the required electronic medical record systems and have used them within the last 6 months.
