Long Term Care Functional Screener

Role: Long-Term Care Functional Screener Responsibilities: Build trusting relationships with IRIS participants, administer and coordinate LTC Functional Screens, document renewals, provide guidance on maintaining funding eligibility, collaborate with team members and community partners, participate in meetings and conferences, prepare reports, testify in hearings, ensure compliance with documentation standards, and strengthen community connections. Qualifications & Skills: Bachelor's degree in social work, psychology, human services, nursing, or related field; minimum 1 year of experience with seniors or individuals with disabilities; valid LTC FS certification or willingness to obtain; bilingual skills a plus; strong organizational, planning, and communication skills; proficiency with smartphones, tablets, and computers; commitment to participant autonomy and meaningful living. Key Requirements: Valid driver's license, reliable vehicle, satisfactory driving record, pass background check, and willingness to travel frequently including overnight trips.
