Join our elite team of healthcare professionals, providing the Right Care, at the Right Time, in the Right Place.**Job Description:**# **Hybrid position: 3 days in office and 2 days remote.**## As a **Transitional Liaison LPN/LVN** you’ll contribute to our success in the following ways:* Identify patients who may qualify for hospice care.* Follow the hospice referral, eligibility, and documentation process.* Assess patients' physical, emotional, social, and spiritual needs.* Work with care teams to coordinate patient transitions and services.* Meet with patients and caregivers to discuss care goals and service options.* Explain hospice services so patients and families can make informed decisions.* Educate staff and referral partners on hospice eligibility and the transition process.* Support the hospice team by coordinating and assisting with admission visits.## ## **What is Required?*** Current state license as Licensed Practical Nurse/Licensed Vocational Nurse* Graduate of an accredited LVN/LPN Program* 3+ years’ experience in a clinical care setting* Experience within Home Health or Hospice is preferred ## **What's in it for you?*** Comprehensive insurance plans for medical, dental, and vision benefits including pet insurance* 401(K) with employer match* Tuition reimbursement for full-time staff and continuing education opportunities for all employees at no cost* Paid time off, paid holidays, family, and pet bereavement* Fleet vehicle program (restrictions apply) or mileage reimbursement.\* Benefit eligibility can vary by position and shift status.
