Job Summary PERM PHYSICAL THERAPIST – Talented Medical Solutions Full-time | In-Office | Fremont, CA, United States Salary range: 56-61/hr Schedule: 35 hours per week, Monday–Friday, no weekend rotation Facility: 126 beds Responsibilities Demonstrated on-site physical therapy treatment delivery per plan of care. Accurate documentation of treatments and outcomes in EHR within 24 hours. Regular progress assessments; adjust plans and record rationale clearly. Interdisciplinary collaboration for care coordination and discharge planning. Strong communication skills to educate patients and families on home programs. Productivity targets with quality-of-care balance; track performance metrics. Proactive participation in process improvements and performance reviews. Qualifications Licensed Physical Therapist with current state license required. Compliance with regulatory guidelines, safety protocols, and ethics.
