Primary Operator

Hartford Healthcare seeks a compassionate Primary Care Nurse to deliver high-quality, patient-centered care in our Healthcare & Medical Services department. In this role, you will perform assessments, administer medications and immunizations, assist with chronic disease management, triage patient concerns, and coordinate care across our network. You will educate patients and families, support preventative care initiatives, and maintain accurate documentation in our EHR. Working within a collaborative, inclusive team, you will use cutting-edge technology, participate in ongoing professional development, and help drive excellence in primary care for our Connecticut communities.
