Registered Nurse (RN) Admissions/Case Manager, Hospice - Fulltime

What you will do: Apply specific criteria for admission and re-certification to establish patient’s eligibility and appropriate levels of care. Assess the impact of the terminal diagnosis on patient’s physical, functional, environmental, safety, ADL and psycho-social needs, including risk for pathological grief. Assess all aspects of the patient’s pain and develop an individualized pain management plan. Identify patient verbal and non-verbal communication patterns. Assess the ability of the caregiver to meet the patient’s immediate needs at start of care and throughout care. Develop an individualized palliative plan of care based on the needs identified which demonstrates management of discomfort and provision of symptom relief. Who you are: Current, active license in the state of Arizona Board of Nursing, pursuant to ARS Title 32, Chapter 15 including fingerprint checks Current CPR, TB, health clearance and clinical skills self-assessment. Successful passage (80%) of Agency competency exam and core competency per skills checklist. Minimum of one year of professional nursing experience and BSN or one year of professional nursing experience within the last 3 years.
