Job Summary:

Seeking an experienced Licensed Practical Nurse (LPN) to provide home health care services. The ideal candidate will have strong wound care proficiency and at least two years of home care experience.

Responsibilities:

• Deliver comprehensive wound care to patients in a home setting.
• Assess patient needs and develop appropriate care plans.
• Administer medications and treatments following physician orders.
• Document patient progress and clinical activities accurately.
• Collaborate with healthcare teams to ensure quality care delivery.
• Maintain compliance with healthcare regulations and safety standards.

Qualifications:

• Valid LPN license with current certification.
• Minimum of 2 years of hands-on home care experience.
• Proven proficiency in wound care management.
• Strong assessment and clinical skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently within assigned zip codes.
