As Director of Shelter and Community Outreach, you will lead outreach initiatives serving families and individuals experiencing homelessness, domestic violence, and other vulnerabilities. You will build strong relationships with shelters, community organizations, government agencies, and local partners while promoting TFH services, coordinating community programs and events, supporting referrals and patient access, and strengthening outreach operations in the Long Island City community.

 

Responsibilities

• Inform shelter directors, social services directors, case managers, staff, and residents about TFH services.
• Maintain open communication with the Department of Homeless Services and shelter organizations.
• Present TFH services to shelter residents, staff, community organizations, and partner agencies.
• Create linkage agreements and coordinate collaborations with homeless and domestic violence shelters.
• Keep shelter partners informed about new initiatives and services for residents.
• Identify organizations and resources that can enhance services for homeless populations.
• Coordinate presentations, information desks, health fairs, and other outreach activities for shelter residents.
• Identify emerging trends among families and individuals living in shelters and participate in coalitions serving vulnerable populations.
• Prepare statistics and reports for upper management.
• Assist patients during visits at TFH and ensure safe, efficient door-to-door transportation for shelter residents.
• Market TFH services throughout the Long Island City community and attend community meetings, political and social functions, and community initiatives.
• Represent TFH on community advisory boards and organize or participate in weekday and weekend community health fairs.
• Organize an annual Long Island City community event, in-kind donations, and the TFH Annual Candy Cane Lane Holiday Celebration.
• Oversee the student internship and volunteer program and support fundraising for TFH as appropriate.
• Conduct ongoing clinic tours and maintain an accurate database or tracking sheet for new client referrals.
• Design and develop the Community Outreach Department procedure manual.

 

Required Qualifications

• Associate’s degree.
• Fluency in English and Spanish.
• Four to five years of healthcare industry experience, with a focus on outreach to vulnerable populations.
• Experience in outreach, community engagement, and working with vulnerable populations.
• Strong organizational and management skills.
• Experience planning and coordinating events.
• Proficiency with EMR, Microsoft systems, and related software.

 

Preferred Qualifications

• Bachelor’s degree.
• Knowledge of and experience with Federally Qualified Health Centers (FQHCs).
