Title
Acute Care Case Manager - Discharge Planner

Schedule
- Weekend requirement: Every other weekend; commit to 3 of 5 major holidays
- Ratios: Days ~1:25; Weekends ~1:45

Requirements
- California state license (pending accepted but must be active by start)
- Minimum 2 years of experience in acute care case management/discharge planning
- Experience with utilization review, care coordination, and discharge planning
- Proficiency with the Epic charting system
- Ability to organize and lead family meetings and participate in treatment planning
- Familiarity with healthcare systems and arranging durable medical equipment, skilled nursing facility admissions, and ambulance transport independently

Roles & Responsibilities
- Coordinate and manage acute care cases, focusing on discharge planning and care transitions.
- Work collaboratively with interdisciplinary teams, including physical therapy and social services, to meet patient needs.
- Conduct assessments to identify complex social needs, including homelessness, substance use, and mental health issues.
- Lead family meetings to discuss treatment plans and discharge goals.
- Maintain compliance with healthcare standards and guidelines, including DRG and CMS requirements.

Compliance / Notes
- Must submit interview availability, confirmed date of birth, and full Social Security number in the system, along with completed skills checklist.
- References: 2 references covering 1 year within the last 3 years (can include Charge RNs/Lead).
- Travel compliance: seasoned travelers only (no first-time travelers). RTO limits apply (7 days).
