Minimum one year's experience. Current state license as a Registered Physical Therapist / PT or be actively in the application process for licensure as an RPT/ PT. Current CPR certification. Duties & Responsibilities The Physical Therapist / PT is responsible for the assessment of referred patients requiring rehabilitation services, providing direct patient care to assess medical condition, functional capabilities, limitations, and potential for rehabilitation. The PT will establish and administer a treatment program with specific goals based on the patient's capacity and tolerance, under direction of the Physician. The PT must set realistic and achievable goals, document and record the patient's condition, and educate patients and families in appropriate physical therapy methods. Develop a plan of care for each physical therapy patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan, evaluate, and modify care to meet goals of physical therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient.
