Social Worker, Hospice

AccentCare is seeking a part-time Hospice Social Worker (MSW) to support patients and families facing serious illness. In this role, you will complete psychosocial assessments, develop care plans, and provide counseling to address emotional, social, and practical needs at end of life. You will collaborate closely with nurses, physicians, and chaplains to ensure coordinated, compassionate care. Drawing on Hanford Pharmaceuticals’ culture of innovation and collaboration, you’ll use evidence-based practices, maintain detailed documentation, and connect families with community resources while upholding the highest quality and safety standards.
