RN Care Manager: Discharge & Utilization Lead

A leading health care provider in Rhode Island is seeking a Registered Nurse Care Manager to perform utilization and case management, ensuring effective patient discharge and transition. Candidates must possess a Bachelor of Science in Nursing and a valid RN license in Rhode Island, with at least 3 to 5 years of clinical experience and knowledge of care management.
