Field Care Manager: LTSS & Care Coordination (Travel Reimbursed)
Molina Healthcare of Illinois
Molina Healthcare is seeking an experienced Care Manager to support Medicaid LTSS members in Northern Cook County and surrounding areas. The role involves in-home visits, care coordination, and building cross-functional plans with the team.
Candidates should have 2+ years in health care, strong Microsoft Office skills, and the ability to navigate complex programs. Mileage is reimbursed; hours are Mon–Fri, 8 AM–5 PM CST.
