Overview Job Description - LPN II - PRN (as-needed) - Primary Care Anacostia (250002SR) LPN II - PRN (as-needed) - Primary Care Anacostia - (250002SR) Description: All nursing practice is based on the legal scope of practice, national and specialty nursing standards, Children’s National Policies, and procedures and in accordance with all applicable laws and regulations. The LPN will provide care under direct supervision of a Registered Nurse, Physician or Advanced Practice Nurse. Children’s National supports the development of the LPN within the community, especially regarding the health and wellbeing of the community at large. Primary Care Anacostia Tech Hours of Operation: Mon - Thurs (8am - 8:30pm), Fri - Sat (8am - 6pm). PRN shifts will vary. Qualifications Minimum Education High School Diploma, GED or equivalent in addition to completion of a 1-year (minimum length) vocational/technical training program in a related field. (Required) Minimum Work Experience 2 years of related experience (Required) Required Skills/Knowledge Ability to perform technical functions interact with children and families and have good verbal and written communication skills. Demonstrates competence in obtaining accurate vital signs, assessment and documentation of patient care. Successfully complete Division of Nursing & Patient Services LPN Medication Administration test. Successfully complete the hospital and nursing orientation. Identify resources and know when to ask for help or escalate. Identify self-learning needs and seek resources to meet those needs. Demonstrate the basic knowledge, judgement, and skill in the delivery of the nursing process, secondary assessment, diagnosis, planning, implementation, education, and evaluation. Successfully complete LPN role if applicable. Required Licenses and Certifications Licensed Practical Nurse (Required Upon Hire). Basic Life Support for Healthcare Provider (BLS) (Required Upon Hire). Functional Accountabilities Clinical Process Differentiate a new problem from a long-term problem at the time of admission/clinic visit; conduct a patient history to elicit information and establish child’s normal baseline routine; respond to inquiries from families by answering questions; explain information clearly and thoroughly. Accurately take TPR, BP, height & weight, head circumference, and intake and output measurements according to procedure. Recognize physical and behavioral changes for groups of patients usually cared for in the assigned area; perform basic nursing care (bath, personal hygiene, dressing, linen change, positioning, bedpan/urinal/diaper change, ambulation); perform therapeutic interventions including tube care, gastrostomy, chest tubes, urinary catheters, IV starts, trach care per protocol, skin care (wound, ostomy) and related procedures; exception: Naso-tracheal/Endotracheal suctioning and insertion of oral/nasal airway. Respond to alarm conditions for feeding/monitoring devices; troubleshoot and escalate as needed; recognize basic changes in child such as apnea, wheezing, retractions, LOC changes, seizure activity, skin lesions, and poor tissue perfusion. Collect and test specimens, pass NG tubes and catheters according to procedure and unit policy. Prepare for monitor setup and D.C.s peripheral IVs after direction from a Registered Nurse. Organizational process Follow infection control protocols and plans. Follow protocols for organizational emergency plans and nursing practice guidelines. Complete a patient assignment and/or assigned task related to screening, intake, admission, discharges & transfers for an assigned unit/clinic; accept patient assignments and collaborate with the registered nurse to implement and document care. Organize workload in collaboration with a co-assigned registered nurse; administer medications or perform assessments/care procedures and contribute to the plan of care with the health care team. Follow physician’s orders and nursing directives precisely; carry out orders immediately within the parameters of the LPN job description. Transformational Leadership Orient children and families to area regulations and routines; prepare all assigned children for examinations, procedures or surgeries; apply growth and development considerations. Review medical records and reinforce teaching outlined by the RN. Teach families how to administer routine medications at home including effects, side effects, and dosage according to guidelines. Implement education plan during hospitalization/clinic visit under RN or physician direction; provide feedback to RN regarding patient/family learning. Escalate patient care when needed and refer to physicians and RNs as indicated. Participate in ongoing education and professional development as required; collaborate with the team to meet patient care needs. Ensure accurate handoffs and cohesive teamwork; assist a float or orientee as needed. Complete mandatory education and regulatory requirements; develop annual goals with the Supervisor to increase understanding of disease and treatments. Exemplary Professional Practice Document observations, interventions, teaching and patient/family responses accurately and timely. Participate in admission/clinic data collection and required documentation; document medication administration with time, route, signature and patient responses where appropriate. Accept patient assignments and collaborate with the nurse to implement and document care; gather and document clinical data and screenings; demonstrate competency in clinical skills within scope of practice. Develop assessment, organizational and prioritization skills to care for patients with changing clinical needs; complete documentation per policy. Provide education proactively and respond to educational needs during hospital/clinic visits. Proactively seek safe medication practices and refills in line with policies when appropriate. Safety Place safety first and respond to distress; institute BLS when necessary. Validate and verify information needed for decision making or documentation; maintain clear and timely communication. Promote safety for patients, families, and coworkers; escalate abnormal findings and reassess vitals as needed.
