Eagle Rock Post Acute Care at Cedar Grove seeks a compassionate Physical Therapist to provide evaluations and evidence-based treatment for post-acute and long-term care residents in our skilled nursing and rehabilitation facility. In this onsite role, you will design and implement individualized care plans to improve mobility, function, and quality of life, while ensuring resident dignity, safety, and comfort. You will document in EMR, collaborate closely with an interdisciplinary team, and educate residents and families.
