What you'll do: Evaluate patients in their homes and develop individualized treatment plans. Improve strength, balance, mobility, endurance, and functional independence. Help patients safely navigate their home environments and prevent future injuries. Educate patients and caregivers to maximize long-term success. Collaborate with physicians and the interdisciplinary care team to achieve the best outcomes. Maintain timely and accurate clinical documentation.

What we're looking for: Active Minnesota Physical Therapist (PT) license. Strong clinical reasoning and independent decision-making skills. Passion for delivering patient-centered care. Excellent communication and relationship-building abilities. Valid driver's license and reliable transportation.
