Medical Social Worker

Day shift, 5 days a week (08:00 - 16:30). Days off will vary and may not be back-to-back.

Floating position in a healthcare facility with variable operations occurring 7 days per week. Caseload of 20-23 patients.

Requirements

• 1 year of acute care experience required.
• California state license not required.
• No more than 7 days of requested time off (RTO).
• No first-time travelers accepted.

Responsibilities

• Provide support and resources to patients and families.
• Conduct patient assessments and develop care plans.
• Advocate for patients' needs within the healthcare system.
• Collaborate with a multidisciplinary team to manage patient care.
• Facilitate patient discharge planning.
