Key Responsibilities of the RN Case Manager

• Complete orientation at Palms West before transitioning to Lawnwood
• Work within a RN/SW/CMA team following a triad model
• Perform discharge assessments using Interqual
• Adapt to various units as assigned

Requirements of the RN Case Manager

• RN Florida/Compact license
• 2-4 years of RN CM experience in an acute care facility
• Experience with Meditech, Midas, Nate, Outlook, and Imobile
• Ability to function independently and adapt to fast-paced environments
