Job Summary
This role supports patients transitioning before and after acute care episodes, focusing on medical, psychosocial, and economic factors. Emphasis is placed on individuals with chronic conditions and functional limitations to help navigate healthcare systems, reduce inefficiencies, and promote coordinated, comprehensive services. The goal is to enhance patient engagement, satisfaction, and overall health outcomes.
Essential Responsibilities
- Assist with initial screening and evaluation for non-complex planned admissions under RN or social worker supervision.
- Facilitate access to services such as skilled nursing facilities, home care, hospice, infusion therapy, long-term acute care, durable medical equipment, and transportation to optimize health outcomes.
- Monitor and reassess care plans for low-risk patients.
- Develop and maintain partnerships with physician practices, outpatient providers, and community services.
- Serve as a liaison for post-acute care providers and physician practices regarding care provision.
- Support system integration and interdisciplinary collaboration to ensure continuity of care.
- Maintain accurate contact information for community partners.
- Manage Medicare communications and observation letters.
- Participate in departmental and community initiatives related to care transitions.
- Assist with utilization review and case management for inpatients and outpatients as needed.
- Ensure accuracy in disposition coding and after-visit summaries.
- Manage department resources, including durable medical equipment loaner closets.
- Handle department communications, including phone and visitor inquiries.
Skills and Abilities
- Strong knowledge of healthcare industry and government regulations related to discharge planning, abuse, advanced directives, safety, and privacy.
- Skilled in promoting patient and family engagement.
- Logical thinker capable of processing complex information.
- Fiscal responsibility in resource allocation.
- Excellent written and verbal communication skills, with computer proficiency.
- Ability to relate effectively to diverse populations including patients, families, physicians, and community representatives.
- Empathy, respect for individual rights, and professionalism in handling sensitive information.
- Calm and professional demeanor in stressful situations.
- Understanding of ethical, legal, and social service frameworks to assist patients in accessing programs.
- Independent decision-making and effective multitasking in a dynamic environment.
- Appropriate use of supervision and consultation.
Education & Experience
- Bachelor's degree preferred from an accredited institution with additional healthcare experience.
- Consideration given to relevant certifications combined with substantial healthcare experience.
Work Shift: 1st Shift
Location: Regional healthcare facility in the Midwest United States.
