Active licensure in New York is required, and EPIC charting experience is a hard requirement.

Responsibilities
- Administer prescribed medications (IV/IM/SQ) and document in EPIC per physician orders.
- Provide direct resident care for adults with dementia, arthritis, CHF, and CVA; monitor vitals; support feeding tube care, wound care, Foley care, and basic oxygen therapy.
- Perform phlebotomy, IV therapy, and other routine procedures; ensure patient safety, comfort, and adherence to care plans.
- Monitor for changes in condition, communicate with Interdisciplinary Team, and implement pressure ulcer prevention and end-of-life care as appropriate.
- Assist with activities of daily living, mobility, and behavioral/de-escalation needs; participate in shift handoffs and ensure accurate charting in EPIC.

Requirements
- Active New York license (RN license or appropriate license in hand) for submittal.
- BLS certification.
- Minimum 1 year of clinical experience.
- EPIC charting experience required.
- Ability to work on a 42-bed long-term care unit with dementia care and related conditions; block scheduling with every-other-weekend coverage; float as needed.
- Non-local candidates outside a 75-mile radius preferred; locals within 75 miles are not accepted.
- Orientation: 16 hours pre-start.
- Pre-approved time off: 1
