Job Requirements / Qualifications

• Active, unencumbered LPN (Licensed Practical Nurse) license
• Minimum 1 year of LPN experience, preferably in long term care, skilled nursing, or LTC settings
• Comfortable and able to work 12-hour night shifts (night shift LPN)
• Strong communication, clinical documentation, and teamwork skills - experience collaborating with RNs and CNAs a plus
• Current BLS/CPR certification preferred

Responsibilities (LPN - Long Term Care)

• Deliver compassionate, resident-centered nursing care in a long term care / skilled nursing facility
• Administer medications and treatments according to physician orders and facility protocols
• Monitor, assess, and document resident conditions; promptly report changes to the healthcare team
• Collaborate with RNs, CNAs, and other clinical staff to ensure high-quality care and continuity
• Assist residents with activities of daily living (ADLs) and promote resident safety and well-being
• Maintain accurate and timely medical records in compliance with facility standards
