CSP Clinical Registered Nurse (RN)

The CSP Clinical Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients. Provides clinical evidence-based services, interventions, education, and consultation for the caregiver and Veteran; community outreach; resource development; continuous quality improvement activities; and the evaluation of caregiver issues. Responsible for the clinical programming and daily operations as assigned.

Functions in support of caregivers, Veterans, and family members throughout the Veterans Affairs Medical Center (VAMC) and in community settings.

Conducts interim and annual home visits to caregivers and Veterans approved in the program and provides assessments via phone and clinical video telehealth.

Provides referrals and/or consults for Veterans Affairs (VA) services, as well as community-based social services and other non-VA entitlement programs. Responsible for developing and establishing linkages and collaborates on VA caregiver support efforts with community healthcare and social service agencies.

Possesses advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state, and private sector resources.

Collaborates with the interprofessional team to manage resources, implement services and benefits to improve Veteran outcomes. Provides and exhibits knowledge in assessment and monitoring, health restoration, wellness promotion, illness prevention, and coping facilitation. Advocates for holistic care through patient/caregiver education and provider interaction and implements an educational plan to meet the patient care needs.

The CSP Clinical RN collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for a group of patients. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
