Summit Home Care & Hospice
Summit Home Care & Hospice is seeking an experienced LPN to join our Hospice team providing direct patient care in home settings and training at the local office or remotely. The role requires an Ohio LPN license and preferably hospice/home health experience.
You’ll participate in on-call rotations and mileage-reimbursed travel within a ~30-mile radius. Responsibilities include implementing the plan of care, documenting accurately, educating patients and families, and coordinating with a
