About This Role As the Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes as prescribed by the physician, ensuring compliance with applicable laws and agency policies. This role is ideal for those who thrive in a collaborative environment and are dedicated to maintaining continuity of patient care. Highlights Pay: $64,132 - $96,198 annual total cash target pay Location: Bainbridge, GA Brand/Division: Optum Schedule: Full Time What You'll Do Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other health care professionals. Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks. Provide required supervisory visits. Deliver high-quality clinical services within the scope of practice and infection control standards. Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely. Document patient visits per policy and payer requirements, syncing timely per LHC policy. Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility. Communicate relevant information timely and effectively with agency staff and other clinicians. Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students. Participate in on-call and weekend rotation as needed to meet patient needs. Requirements Education Current and unrestricted RN licensure in state of practice Experience 1+ years of Home Health experience preferred Licenses & Certifications Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation Current CPR Certification or ability to complete within 90 days of hire preferred Benefits Comprehensive benefits package Recognition programs Equity stock purchase 401k contribution About UnitedHealth Group UnitedHealth Group is a leading healthcare provider committed to delivering exceptional patient care.
