Home Health Physical Therapist

Home Health Physical Therapist needed for a home health setting. The position involves visiting patients in various locations within a specified radius. Therapists may visit up to 6 patients per day. A productivity standard of 30 points is established. No weekend work is required. The facility does not accommodate alternate work schedules such as 4x10s or 3x12s. The traveler will not need to float between units and is not on-call. The charting system used includes Epic Remote Client for Home Health and Rover. Business casual attire is acceptable, and scrubs are allowed but not required. The system is designed to minimize drive time between patient visits. A maximum driving distance may reach roughly 60 miles per day. Oasis documentation is required. References are needed covering the last year. Travelers will not be expected to float between facilities. Candidates should not contact facility personnel directly for inquiries. Module completion for training is non-billable and is calculated within hours worked. Specific scheduling requests must be discussed and confirmed during the application process.
