Responsibilities
- Provide direct patient care to residents in a long-term care setting, under supervision of a registered nurse
- Administer medications and document administration accurately in the charting system
- Monitor residents’ conditions, measure vital signs, and report changes promptly
- Assist with activities of daily living and basic wound care, while promoting resident comfort and safety
- Collaborate with the care team, including RNs, physicians, and certified nursing assistants, to implement and adjust care plans
- Maintain accurate and complete documentation, follow facility policies, and ensure compliance with resident rights and safety standards

Requirements
- Current LPN license in the state of New Mexico (active)
- Graduation from an approved practical nursing program (Diploma or equivalent)
- Prior experience in a long-term care or skilled nursing setting preferred
- Ability to work night shifts (18:00–06:30) and maintain patient safety and quality of care
- Effective communication and documentation skills; able to collaborate with interdisciplinary team
- Must be able to work in a long-term care environment and meet physical and cognitive demands of the role
