SUMMARY : The Nurse works collaboratively with providers and support staff to ensure the smooth and efficient running of the Same Day Care Department.

SPECIFIC DUTIES AND RESPONSIBILITIES : All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA).

• Processes all patients according to the policies and guidelines established for Same Day Care.
• Ensures continuity of care is maintained in all aspects of patient care.
• Responsible for data collection systems and to report statistical data necessary for grant purposes as needed.
• Supports all quality assurance activities and infection control procedures according to established policies.
• Assists in maintaining stock inventory and ordering supplies, as necessary.
• Attends department, case management and other meetings as assigned.
• Maintains strict confidentiality at all times.
• Performs other duties and tasks as assigned.

Qualifications :

• Experienced and able to function effectively and professionally within an outpatient setting providing community-based care.

Education and/or Experience :

• Graduate of an accredited nursing program
• CT Licensed Practical Nurse (LPN) license required or Registered Nurse (RN) license required

  Physical/Work Environment Requirements:

• Variable 12-hour shifts (3 days a week) including weekends (Saturdays & Sundays) and holidays
• Minimal physical effort
• Must be able to operate computer and telephone continuously

Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.

In addition, this position will be eligible for additional benefits consistent with the role. The compensation of the finalist selected for this role will be determined based on various factors, including but not limited to: scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation. The hiring range listed is a good faith determination of potential compensation at the time of this job advertisement and may be modified in the future.
