This is a 13-week contract position.

Job Description

We are seeking an experienced Case Manager to provide exceptional care in an acute hospital setting. The ideal candidate will have a strong understanding of care coordination and discharge planning while ensuring compliance with CMS guidelines and HIPAA regulations.

Responsibilities

• Conduct needs assessments and develop plans of care for patients.
• Coordinate care across multiple disciplines and services.
• Manage prior authorizations and appeals related to patient care.
• Determine medical necessity per evidence-based guidelines.
• Provide education and support to patients and families.
• Ensure compliance with regulatory standards and hospital policies.

Required Experience / Certifications / Licensure

• Active MI RN license is required.
• 2 years of acute hospital experience preferred.
• Epic experience is required.
• Previous charge experience is a plus.
• No call requirements, but must float to any inpatient unit as needed.
• Availability to work one weekend per month, with a weekend shift replacing a weekday shift when scheduled.
