• Full Time Day Shift

• Long Term Care Experience 1-2yr Required

Now Hiring: LSW Licensed Social Worker

Responsibilities:

• Identifies cognitive impairments, mood problems and other psycho-social needs in residents.

• Maintain accurate, thorough, and timely documentation.

• Review Advanced Directives and provide education to residents and family members as needed.

• Help provide a smooth admission and discharge process for residents and family members.

• Other duties as assigned by Administrator.

Qualifications:

• Ability to work independently or part of a group.

• Computer knowledge.

• Ability to build and maintain rapport with residents and peers.

• Prior Long Term Care/Skilled Nursing Experience in Social Services or Required

• LSW Certificate required
