Optimize patient care transitions and resource utilization in a premier academic medical center. Lead utilization review, discharge planning, and interdisciplinary coordination for high-acuity patients while ensuring compliance with EPIC documentation standards.
Competitive Compensation: $65/hour

Work Schedule: Monday - Friday (8:00 AM - 4:30 PM (Day Shift))
