Veterans Housing Stability Case Manager

Catholic Community Services is seeking a Case Manager for the Veterans Services program serving nine counties. You will manage a caseload in the Veterans Homeless Prevention/Rapid Re-housing program, perform intakes, assess housing needs, and assist with financial housing costs. You will develop Housing Stability Plans with VA/community resources, enter data into HMIS, and coordinate with the Veterans Program Manager to deliver strong case management.
