What You'll Do

• Conduct occupational therapy evaluations and provide treatment within patients' homes
• Create personalized care plans designed to promote independence
• Instruct patients and caregivers on safety techniques, adaptive methods, and equipment use
• Complete documentation in a timely and accurate manner
• Share updates on patient progress with the care team across disciplines
• Ensure adherence to all clinical guidelines and documentation requirements

What We're Looking For

• Active Occupational Therapist (OT) license
• Prior home health experience is a plus, but not mandatory
• Excellent verbal and written communication abilities
• Capable of working autonomously while effectively managing your schedule
