Responsibilities

• Evaluate and develop individualized occupational therapy treatment plans
• Help patients improve skills for daily living, self-care, and functional independence
• Provide interventions for patients with physical, cognitive, or psychosocial challenges
• Collaborate closely with PTs, SLPs, nursing staff, and families for holistic care
• Document progress accurately and timely to support quality care and compliance

Qualifications

• Licensed Occupational Therapist (OT) in the state of CA or ability to obtain license
• Experience in post-acute or SNF setting preferred, but new grads are welcome
• Strong communication, collaboration, and problem-solving skills
• Passion for patient-centered rehabilitation
