Nurse Practitioner (NP)

The position is based onsite in Surprise, Arizona and reports to the Associate Medical Director. Responsibilities You will provide comprehensive, primary care to our older adult population. You will manage patient care, including both acute and chronic care management, prevention, and patient education. Required Qualifications Active, unrestricted nurse practitioner license in the state of Arizona Board certification Proficiency with electronic medical records (EMR) 2 years of experience working with geriatrics 2 years of primary care experience Schedule and Compensation Scheduled Weekly Hours: 40 Pay Range: $123,800 - $170,300 per year. This job is eligible for a bonus incentive plan based on company and/or individual performance.
