Hospice RN Case Manager — Home-Based Care & IDT Lead

Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and provide clinical, palliative and home-based care to keep patients in their homes as long as possible. The role requires an active RN license in Florida, at least 1 year of hospice or hospital experience, EMR/EHR proficiency, and a valid driver’s license with reliable transportation.
