Registered Nurse - Case Management

Location: Pasadena, TX

Assignment Duration: This is a 13-week contract position.

Schedule: Shifts are from 20:00 to 17:00.

Job Description: As a Registered Nurse in the Case Management Department, you will be responsible for coordinating patient care activities across the hospital. Your primary focus will be on discharge planning and ensuring compliance with Interqual criteria to confirm patient status.

Responsibilities:

• Coordinate discharge planning for patients
• Utilize Interqual criteria to assess patient statuses
• Collaborate with a multidisciplinary team to ensure patient needs are met
• Maintain communication with patients, families, and healthcare providers

Required Experience / Certifications / Licensure:

• Must possess a valid RN license
• Minimum of 2 years hospital case management experience is essential
• Strong communication and organizational skills required
